• End Date: December 20, 2025\n
• Duration: 12-week nursing contract\n
• Shift: 8-hour day shifts, 08:00 AM – 05:00 PM\n
• Guaranteed Hours: 40 hours per week\n
• Weekend/Holiday Coverage: May be required (Sunday–Saturday, including holidays)\n
• Estimated Weekly Salary: $2,560 - $2,750\n

\n\nRegistered Nurse Qualifications\n

\n
• Active RN license in Oregon, Washington, or compact state (required for travel nursing jobs in Oregon)\n
• Minimum 1 year of experience as a Patient Care Coordinator RN or Case Manager\n
• BLS certification (American Heart Association)\n
• Proficiency with EPIC electronic health records (EHR) system\n
• Knowledge of Medicaid and Medicare processes\n
• Experience in outpatient nursing or staff lead roles preferred\n
• Previous employment with large healthcare systems is a plus\n

\n\nPatient Care Coordinator RN Responsibilities\n

• Coordinate patient care activities and serve as a clinical resource for nursing staff in the outpatient setting\n
• Facilitate communication between patients, families, and the healthcare team\n
• Ensure compliance with individualized care plans, documentation standards, and regulatory requirements\n
• Utilize EPIC for accurate and timely charting and patient records management\n
• Assist patients with Medicaid and Medicare navigation and resource access\n
• Support scheduling, patient flow, and process improvement initiatives in the clinic\n
• Provide leadership, mentorship, and clinical support to nursing and allied health staff\n
